Advice and guidance on staying healthy

How to get information, advice and guidance from the Prevention Assessment Team (PAT) on how to stay healthy and well.

1 Who are we

The Prevention Assessment Team (PAT) is a collaboration of partners from West Sussex County Council, Sussex Community Foundation NHS Trust (SCFT) and Age UK West Sussex Brighton & Hove providing information, advice and guidance to people aged 18 years and over.

We are here to help residents across West Sussex access the support they need to live independently at home and enjoy life as part of their community.

We are also here to help prevent or delay the need for more intensive services. We normally work with people who do not receive services from specialist health-care teams or social care services. 

2 How we can help

This means we can:

  • check your general state of health and wellbeing
  • help you identify what may benefit you to retain your independence
  • enable you to make any changes
  • let you know what resources may be useful to you
  • provide assistance to access practical and emotional support

For example, one of our team could:

  • advise you about how to keep healthy and manage health issues
  • link you with agencies that can maximise your income and help you remain independent through practical tasks such as shopping, cleaning and gardening
  • put you in touch with local social groups and activities such as art classes, learning courses and sewing clubs
  • help you keep your home safe, secure and warm
  • let you know what transport services are available locally

Our nurse advisors attend and set up community events offering blood pressure and pulse checks. We also provide NHS health checks in outreach settings by appointment. 

3 How to get in touch?

Contact us if you have any questions or think we may be able to help you or someone you care for.

We are available Monday to Friday, except on bank holidays. If you phone outside these times, leave a message on our answerphone and we will phone you back as soon as we can.
