A message from the executive director, director and head of service

The Combined Placement and Sourcing Team (CPST) is a growing and developing provision within our Adults’ Services at West Sussex County Council with exciting plans for the future.

The CPST was established in April 2020 as part of the council’s response to the COVID-19 pandemic to support people to be rapidly discharged from hospital.

We are now building the service, which provides an important part of the way we support residents of West Sussex as we recover and move forward from the pandemic.

The CPST provides a central point for sourcing care and support for adults, working in a strengths-based way to help people maintain or regain independence.

The next stage is to bring in sourcing of specialist services for people with lifelong disabilities or requiring support with their mental health, to support them to live a good life.

CPST is made up of seven teams, each focusing on different customer groups or specific services. All of our teams put our customers at the centre of everything they do, working creatively to support people to maintain their independence at all stages of their life.

The teams are:

  • Residential team - This team supports older people in sourcing the right care home to meet their needs in the best way
  • Non-residential teams (northern, southern and western) - These three teams support people to remain in their own home by accessing a range of services
  • Hospital discharge team - This team works with people requiring support to be discharged from a general hospital
  • Data team - A team that supports the work of our service by ensuring our information and records are accurate and processes are efficient
  • Lifelong services (LSS) and Woking Age Mental Health Team (WAMHS) - A new team being developed to provide services to support people with a lifelong disability or who may have a mental health condition to plan and access the right support at the right time
