About us

The Combined Placement and Sourcing Team (CPST) is a growing and developing service with exciting plans for the future. We would love you to be part of this.

The CPST was a brand new team established in April 2020 as a vital part of our COVID-19 response to support local communities and residents. They work across health and social care to ensure that people in hospital are effectively and supportively discharged. This gets people back to their lives and the place that they call home in the timely way that they need.

We also ensure that people with longer-term needs are looked after and receive the services they need to live good, independent lives.

The CPST works closely with social work teams, hospital discharge teams, partner organisations and service providers to ensure that people receive the most effective support. They play a pivotal role in sourcing services and work with providers to enable people to access the right support that:

  • builds on their strengths
  • keeps them safe and well
  • maximises their levels of independence.

This is a highly valued team and we are proud to have facilitated over 11,000 hospital discharges since April 2020. We have also supported over 4,600 people to meet their assessed social care needs.

Roles supporting hospital discharge involve working in teams on a rota basis, which are planned in advance, to support people needing support to leave hospital. This may involve evening and weekend working.

Roles supporting adult social care are required during standard office hours usually between 8.00am to 6.00pm.

The salaries below show what you can expect:

  • Placement and Sourcing Assistant - £20,092-£20,493
  • Data Officer - £20,092-£20,493
  • Care and Support Broker - £23,080- £25,481
  • Placement and Sourcing Deputy Lead - £29,577-£32,234
  • Placement and Sourcing Lead - £36,922-£39,880

Additional allowances may apply to roles which work on a rota basis covering evenings and weekends.

About you

We are looking for people to join our service who display the right attitudes and skills to be able to support people, whatever their age and stage of life. This includes having the ability to:

  • listen and build trusted relationships with people, keeping them at the centre of everything we do
  • understand people's individual needs and how best to support them
  • work closely with colleagues across health and social care and with service providers to plan and arrange the delivery of strength-based and customer-centred services
  • respect the dignity and diversity of the people you work with
  • communicate to a wide range of people ensuring that information is relayed accurately and understood
  • help keep people safe by determining risks and communicating safeguarding and service quality issues
  • engage in critical reflection with a desire to keep on learning and improving our service.
