We have several Leaving Care teams spread across the county, based in Bognor Regis, Worthing and Horsham.

The Care Leavers service work with our Care Leavers between the ages of 16.5 to 25 in their transition to adulthood.

The teams support Care Leavers to develop the skills needed to live independently and be aspirational. They help them identify and achieve their goals, including healthy living, stable homes, good education, training, and work.

Young people moving into adulthood face challenges. This is even tougher for Care Leavers, who have often faced trauma. As a result, they may need support with mental health, substance use, and behaviour issues.

Roles that support Care Leavers

These teams have several roles, like Support Workers, Housing Support Workers, and Personal Advisors. Below you will find a little more information on what the roles entail.

Personal Advisors

Personal Advisors work directly with Care Leavers who are transitioning to adulthood and leaving the care of the local authority.

Through needs assessments and pathway planning they work in partnership to empower and support our Care Leavers to achieve their goals and aspirations and provide additional support and or crisis intervention when required.

They work collaboratively with other agencies and services to secure good outcomes.

Support Workers

As a Leaving Care Support Worker, you will work alongside Personal Advisors in engaging with our Care Leavers to provide practical support and guidance.

Your role may include helping them to access and attend meetings with a range of services including solicitors, housing, mental health services, employment, education and Department of Work and Pensions (DWP).

You will support Care Leavers looking at:

  • opportunities for employment
  • training
  • education
  • maintaining and sustaining housing
  • budgeting.

Housing Support Worker

As a Housing Support Worker, you will work alongside Personal Advisors and Support Workers to support Care Leavers to understand their housing options and in meeting their housing needs and aspirations.

You will support those who are at risk of becoming homeless and respond to those who require urgent support to avoid this. You will work collaboratively with housing providers and colleagues to secure good housing pathways for our Care Leavers.
