Online form to be used by the Customer Service Centre to report school closures.

School * Select the name(s) of the school.

Date of closure * Select the closure date.

Closure status * Select if the school is closed or partially open.

Description Add any additional details, such as reason for closure. This will appear on the live website for parents.
