This is an NHS service managed by the NHS Surrey and Sussex Integrated Care Board.
Some children and young people have very complex health needs, which cannot be met by existing GP, hospital or community services.
Ongoing support from CYPCC is offered to children and young people who have had a full assessment of their health needs and meet the Children's Continuing Care criteria. You/your child will be asked for consent for the assessment process to begin. The assessment involves sharing information between health, care, education and other services involved with your child.
If you think that your child should be assessed for Children's Continuing Care, talk about this to a health or social care professional that knows your child. If they are likely to be eligible, they will refer your child to the team.

What we do
- CYPCC is for children and young people up to their 18th birthday. After that, the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care is used. The services are different and the team will support you and your child from one service to another before they turn 18. You can find out more about this transition at the website link above.
