You told us that GP and hospital appointments can be difficult and stressful.

It’s often hard to explain what helps you or your child with SEND so they can get the right medical care.

To make things easier, NHS staff worked with Parent Carer Forums in West Sussex and East Sussex. Together, they created information about health and care passports to support families.

How a health and care passport can help

A health and care passport is a document which shares key information, including:

  • any diagnoses
  • how you communicate
  • what support works best

A health and care passport should prevent you having to explain things again and again at appointments.

Complete and print your health and care passport at your local library

It is helpful to have paper copies of your health and care passport.
