Children and Young People’s Continuing Care
Information about healthcare for children and young people with very complex health needs.
Ongoing support from Children and Young People’s Continuing Care (CYPCC) is available for children and young people who:
- have had a full assessment of their health needs
- meet the CYPCC critera
Talk about getting an assessment with a health or social care professional who knows your child. They can refer your child to the CYPCC team.
If eligible, support is available through CYPCC until the child's 18th birthday. The team support your child's move to adult NHS services after they turn 18.
