Lost library cards

Report a lost library card.

This is a manual service that operates Monday to Friday 9.00am - 5.00pm and we aim to complete requests as quickly as possible.

  • Please allow yourself enough time to complete this form in one session as there is not an option to save and come back to it later.
  • The questions marked with an asterisk (*) are mandatory and require an answer. You do not have to answer all of the other questions, but it would help us if you do.
  • This form notifies us to cancel your card. This will stop anyone – including yourself – using it to obtain or renew items.
  • If you are reporting a lost card on behalf of a child, please use the child’s details when you complete the form instead of your own.
  • We will update any account details you supply on this form; if you provide a new email address we will copy in your current email address in our response for security purposes.
  • You can visit your local library (with identification) on completion of this form to be issued with a new card.

Your details

Enter your first name.
Enter your last name.
Enter your address name or number. This could your flat, house or building name or number.
Enter the first line of your address. This could be your street name.
Enter the second line of your address if you need to. This could be your village name.
Enter the town or city of your postal address.
Enter your postcode, with or without spaces.
Enter an email address where we can write to you.
Enter a daytime phone number that we can ring you on. This can be a mobile or land line number.
Enter your date of birth using numbers following the format dd/mm/yyyy, for example 01/01/2015.
