Summer Reading Challenge volunteer application form

Use our online form to apply to be a volunteer for this year's Summer Reading Challenge.

  • This form will take approximately 5 minutes to complete.
  • Please allow yourself enough time to complete this form in one session as there is not an option to save and come back to it later.
  • The questions marked with an asterisk (*) are mandatory and require an answer. You do not have to answer all of the other questions, but it would help us if you do.
  • The Summer Reading Challenge volunteer role is suitable for people aged 14+.
  • Find out more about being a volunteer by reading the role description (opens in new window).
  • We ask all applicants to obtain a reference (opens in new window) from an adult who knows them, but is not related to them (for example, a teacher or tutor). If you are under 16, you will need to ask a parent or carer to complete the consent form (opens in new window). Please bring both these forms with you when you visit the library for the volunteer information session.

Enter date of birth using using numbers following the format dd/mm/yyyy, for example 01/12/1980.
If you attend school or college, enter the name.
Enter the name of someone we can contact in an emergency (for example, a relative).
Enter your emergency contact's phone number.
Enter any health conditions you think we should know about.

Which library would you like to volunteer at?

We will try to place you at your first choice library but we aim to provide volunteer support at all our libraries and sometimes we might need to ask you if you can help elsewhere.

We will be holding information meetings at libraries across West Sussex in June and July 2017. Please select the meeting you would like to attend.
