The questions marked with an asterisk (*) are mandatory and require an answer. You do not have to answer all of the other questions, but it would help us if you do.

Please note: We only book notices for marriages and civil partnerships taking place within the next 4 months. If your ceremony is in more than 4 months’ time, please contact us closer to the date.
