Social work for adults with learning difficulties and disabilities
About the social work teams
There are 4 social work teams for adults with learning
difficulties.
- 3 are part of Community Teams for People with
Learning Difficulties (CTPLDs) – these are joint teams
with Sussex Partnership NHS Foundation Trust and are based in
Horsham, Worthing and Chichester.
- The other team is the Challenging Behaviour
Commissioning and Support Team (CBCST). This is an
integrated team managed by West Sussex County Council, comprising
of Social Workers and Care Managers, employed by West Sussex County
Council, and Speech and Language Therapist, Psychologist and
Behaviour Support Practitioners, employed by Sussex Partnership NHS
Foundation Trust. This team has close links with
colleagues working in
CTPLDs
.
The social work teams are responsible
for assessments and arranging services for all those with learning
difficulties who are aged 18+. This is done in line with the
principles of self-directed support.
They also work with young people aged 16+ and their families to
plan for transition to adult
services.

Health staff within the community teams
These staff are employed by Sussex Partnership NHS Foundation
Trust.
They include:
- a community nurse manager/team coordinator and community
nurses;
- a consultant psychiatrist and associate specialist;
- occupational therapists;
- speech and language therapists and assistants;
- physiotherapists;
- clinical and counselling psychologists; and
- admin support.
The health staff, in addition to offering specialist interventions,
support adults with learning difficulties to get treatment from
primary and other specialist health services.

Who the teams work with
These teams will only work with customers who need a specialist
service due to their learning difficulty. As a rough guide, this is
people with an IQ of less than 70 and whose social functioning is
significantly impaired.
There are many people in West Sussex who have a mild learning
difficulty or specific learning difficulties, such as difficulties
in reading and writing, who do not need this specialist
service.
All new referrals to the Learning Difficulty Service in West
Sussex are assessed by health and social care workers. For people
who do not need a specialist service, advice and support with
making contact with other services is offered.

Team priorities
All areas of need are important, but the key priority areas
are:
- adults' safeguarding and/or serious harm to the person or
others;
- preventing placement breakdown and providing support if
anything does go wrong;
- hospital discharge;
- transition from children's services to adults' services;
and
- people with older carers.

Getting a service
Referrals are welcome from individuals or their carers, families
or representatives. These can be made by letter, phone or in
person. However, referrals for psychiatry must come through the
person's own
GP
. Our
offices are open Monday – Friday, 9am – 5pm. We also provide an
emergency helpline service out of office
hours.
