Prevention and wellbeing support

Prevention, wellbeing and healthy living often go hand in hand. Healthy living goes beyond - eating a balanced diet, taking regular exercise and avoiding illness. It also reflects the emotional and social aspects of an individual's life and is about making informed choices and remaining in control of your life.

The Prevention and Wellbeing Programme in West Sussex will improve access to services for all adults. We also hope to improve people's awareness and knowledge of health and wellbeing.

Information relating to local groups and support within West Sussex can be found on the Wellbeing website and directory. Your local Wellbeing hub can also provide information.

If you require information to help identify what support might be available to keep an individual or their carer independent and prevent or delay them requiring more intensive services, please contact your local Prevention Assessment Team.

  • The Prevention and Wellbeing Programme includes a 'hub' function, which provides and coordinates services that improve the health and wellbeing of local residents.
 
 
  • Prevention Assessment Teams (PATs) aim to help people living locally to stay healthy and remain independent. Normally, they work with people who do not receive services from specialist health care teams or social care services.
