Social care
Social care workers support children in
their families and communities through:
- Care management area teams
- Child disability teams (CDTs)
- Children's occupational therapy teams
Care management area teams
West Sussex is divided into 3 care management areas.
Each area team works with children with additional needs, but
not those with complex disabilities. You can contact your local
care management area team in one of the following ways.

Child disability teams
Child disability teams (CDTs) provide a social work service to
children who have severe and/or complex disabilities and their
families. They can offer advice and assess your child’s needs and
your needs as a parent carer and discuss possible services with
you, including:
- care in your home;
- short breaks; and
- activities for your child.
Staff will carry out an initial assessment of you and your child’s
needs. An in-depth assessment will be carried out if you are
eligible for assistance. In the event you are not eligible, the CDT
may be able to point you in the direction of other organisations
and services that can help.

Children's occupational therapy teams
Children’s occupational therapists (OTs) assess children and
recommend ways to help them to be as independent as possible at
home.
For example, they may:
- suggest equipment that might help your child at home
- advise on adaptations to your home to make caring for your
child easier and safer
- assist with getting ramps, handrails and other equipment
- help you to apply for grants that may be available
