NQT registration and induction assessment forms

The registration of a Newly Qualified Teacher (NQT)

Once a Newly Qualified Teacher (NQT) has been appointed, the headteacher must register them with the Appropriate Body in order for the induction period to start. The registration should take place in advance of the NQT commencing employment, to ensure that the induction period can start straight away.  

The Local Authority acts as the Appropriate Body for all schools in West Sussex for which it has responsibility and is required to keep a record of the names, Department for Education reference number and date of birth of each NQT for whom it is the Appropriate Body. It is also required to keep a record of the stage of the induction support programme which each NQT has reached, based upon information from headteachers. Schools must supply this information for each NQT employed in their school, including those working in the school as a supply teacher for one full term or more.

The Appropriate Body is also required to notify the General Teaching Council of any NQT who starts an induction period.

The assessment of an NQT

Following each of the first 2 formal assessment meetings, an assessment form should be completed by the headteacher (or the professional tutor/induction tutor/mentor acting on behalf of the head teacher). These reports should clearly indicate whether or not, at the time of each assessment, the NQT is judged to be making satisfactory progress towards completing their induction support programme successfully.

In the section entitled 'NQT's comments', NQTs should be encouraged to add their own comments on the summary of progress presented in the form.

The form must be signed by the headteacher, the mentor and the NQT.

Completed assessment forms

A copy of the completed form should be sent to the Appropriate Body within 10 working days of each summative assessment meeting.

The original form should be given to the NQT and a copy kept in school.


N.B.  It is a statutory requirement that
copies of all assessments and supporting evidence should be retained for 6 years by the school.
