Community partnership teams (CPTs)
Six community partnership teams (CPTs) for older people bring together specialists who can advise or actively put in place measures to support an older person’s health, quality of life, and ability to live independently. They usually support people who are not already receiving formal services from specialist health teams or social care services.
The teams consist of:
- health advisors
- social care workers
- advisors from the pension service
- support workers from local charities
- volunteers
They can:
- check your general state of health and wellbeing
- advise you how to stay healthy or manage health issues, such as your diet, giving up smoking, or taking up exercise
- help you identify what may benefit you
- provide assistance to access practical and emotional support
- help you maximise your income and answer your benefit or pension queries
- link you with agencies that can help you with practical tasks, such as shopping, cleaning and gardening
- help you to access groups and activities, such as lunch clubs or hobby groups
- help you to keep your home safe, secure and warm
